Primary health care and community health programs are offered by an interdisciplinary team of professionals. The team includes:
doctors, nurse practitioners, nurses, a dietitian, a physiotherapist, respiratory therapists, chiropodists, a social worker and a
community health worker. Community health programs emphasize prevention, health promotion and education. They include
healthy eating, counselling, hearing screening, flu clinics, smoking cessation, diabetes clinics, foot-care, and chronic disease
self management. Currently, there is a waiting list to access primary care services. However, anyone who lives within the
catchment area can participate in programs offered by the community health centre. Our catchment area includes the
communities of Merrickville-Wolford, as well as portions of North Grenville (former Oxford on Rideau), Elizabethtown-Kitley,
Augusta, and Montague Townships, and the south west corner of the City of Ottawa (Burritt’s Rapids Village).
